In Follicular Unit Extraction, each graft is derived directly from the head or body using a round punch device.  Slits are carefully placed in the bald regions. And grafts are inserted into each one. With this technique, there is no resulting scar. The extraction phase does result in tiny round wounds. But they will heal within a period of weeks.

The older method for hair transplant is strip surgery. Doctors  remove a horizontal strip of scalp tissue that has healthy growing hair.   This can only be safely done on the back or sides of the head. Not only does this restrict the type of hairs that can be transplanted, it also results in a large scar. Patients often feel compelled to wear their hair longer than they would like in order to hide this.

Follicular Unit Extraction techniques can be applied to the crown area to restore hair density in this region. Some patients may not have a large enough donor supply on their head.  In these cases, areas of the body can supply donor follicles. For example, UGraft FUE techniques can be applied to extract grafts from the beard area.

However, this patient did not require a body hair transplant.

Transplanting hair in the crown area requires a very detailed approach. Doctors must study the existing growth patterns on the individual patient’s head. This includes paying attention to factors like:

(1) growth angle

This is the angle that the hair grows from the surface of the scalp. It is never 90 degrees.

(2) orientation

This term refers to the arrangement of hairs with respect to each other.  For example, they do not grow in a straight line. In the crown area they form a whorl pattern in most patients.

Although hair in the crown is missing, doctors must be able to recreated the whorl on the back of the head.  This arrangement needs to blend in with the hair growth patterns on the rest of the head. Otherwise,  the hair transplant results will look strikingly different in the crown area compared to the remaining area.

Here are the results of this patient’s crown restoration using a donor pool of 1200 grafts. This supply was able to give adequate coverage to this area. And the patient was happy with his outcome.

In Follicular Unit Extraction, patients do not have to worry about a large scar in the end. Each graft (a natural grouping of 1-4 follicles, known as a Follicular Unit) is derived directly from the scalp. Sometimes these extractions are performed on other donor regions on the body, such as the face and chest.

By contrast, the older method of strip surgery requires a horizontal excision of scalp tissue.  This is further dissected into individual grafts that are inserted into the bald regions.  A large scar will result from this type of hair transplant procedure. And patients usually wear their hair in longer styles to hide this.

With Follicular Unit Extraction, individuals can choose long or short styles since there are no scars. After the follicular grafts are extracted, there are usually small round wounds about the size of a pencil point.  However these will usually heal completely in about several weeks.

Crown Hair Restoration

When it comes to crown hair restoration, the doctor must determine the natural pattern of growth for the individual patient.  This includes determining the center of the whorl, and creating slits according to the right orientation and growth angle.  The grafts are then inserted into these slits.  By paying proper attention to these growth patterns, the results of the crown restoration will be inconspicuous compared to the rest of the hair.
