<p>The patient presented in this photo had an abnormal hairline scar and recession as well as an abnormal hair angle. This problem resulted from a Juri flap procedure that he had undergone several years prior to finding Dr. Umar to repair his poor hair transplant. His hairline and remaining problems with his previous hair transplant were repaired by Dr. Umar using only 2050 grafts that were derived from the head and nape donor hairs using his advanced version of Follicular Unit Extraction, which he calls Single Follicle Extraction and Transfer (FUE-SFET). Flap surgeries for the purpose of hair restoration and hair replacement use to be commonly performed in the past, but with the advent of follicular unit hair transplantation, flap surgeries have been becoming obsolete. Technical terms that describe these flap surgeries include Temperoparietal-occipital (aka Juri flaps or Fleming Mayer) and temperoparietal flaps (Elliot flaps).</p>
<p>The Juri flap operation relocates large areas of dense, growing hair. Problems usually associated with the results of these surgeries are mostly found in the esthetics of the hairline. Commonly, the hairline is too straight, defined by a scar, a backward directed hair angle, and abnormally blunted temporal recesses. The sharp hairline makes an unnatural transition from the hairline to the temple points and the hairline symmetry is too abrupt and abnormal. The Juri flap causes the hair to point in the wrong direction (to the sides and backwards) and thus when balding progresses, it leaves a big hole in the balding area, which makes it hard to conceal.</p>

<p>The patient had strip surgeries with large plug transplantation to his frontal scalp. However, as his hair loss progressed, the transplanted hair was still intact due to its permanent nature. The result of this situation is an abnormal pattern in which the patient has an island of hair in the front surrounded by areas of hair loss. The patient came to have this abnormal hair loss pattern repaired by Dr. Umar in early 2008. About 3000 grafts were transplanted by Dr. Umar using his advanced form of Follicular Unit Extraction method (FUE), which is a method that he calls Single Follicle Extraction and Transfer (SFET). This is a knife-free method that is minimally invasive and leaves no linear scar and is less painful than the traditional strip surgery method. Dr. Umar was able to use 3000 grafts taken from the patient’s nape and head to cover the crown and bridge the unnatural gap between the isolated tuft of hair in the front and the side and temples.In conclusion, patients who receive hair transplants at an early age need to remember that the new hair they are transplanting to their heads is permanent, which will lead to aberrant hair loss patterns in the future due to the progression of unstable and unpredictable hair loss that continues with age. <p>The patient reported that all aspects of his follicular unit extraction (FUE) procedure in Mexico were performed by technicians instead of the doctor. This is can lead to poor surgical results because follicular unit extraction requires a very high skill level of the hair surgeon. There is very little margin for error each time a follicle is extracted. That is why in the state of California, any skin incision is considered surgery, hence a surgical act occurs with each of the hundreds or thousands of grafts that is incised each time a single hair follicle is scored in the process of FUE. This procedure differs from follicular unit strip surgery (FUSS), where a surgical act occurs only during the “cutting out” process of the strip of flesh and suturing. Thus, while it is negligent for a doctor to perform FUE without the necessary skill and training, it is illegal in many developed parts of the world for non-licensed persons to be performing surgery. Additionally, a bad outcome would deplete the donor hair supply from wastage of the follicles.</p>

Many of these conditions are inflammatory in  nature. Some conditions, if not recognized and treated early,  could result in scarring with permanent hair loss. Hair loss with scarring (cicatricial alopecia)  is often the result of this condition, and if it is left unchecked, it  could lead to disfiguring lesions, especially in the scalp and face.</p>

Scarring alopecia divides  into two categories: primary and secondary. In primary scarring  alopecia, the hair follicle is the main target of inflammation. In  secondary cicatricial alopecias, destruction of the hair follicle is  incidental to a non-follicle-directed process or external injury, such  as severe infections, burns, radiation, or tumors.
